"Brown was an enthusiastic angler and in 1750 at the suggestion of Dr. Johnson brought out an edition of Walton and Cotton's "Compleat Angler". . . MOSES BROWNE 1704 – September 1787 was a pen-cutter from Clerkenwell London England who became a poet and eventually rose amongst the ranks of the Church of England. He made various contributions to the Gentleman's Magazine founded by Edward Cave at St. John's Gate in 1731 who awarded Browne several prizes for his contributions. Browne mixed with some distinguished literary figures of his time including befriending Samuel Johnson. Browne was appointed vicar of Olney Buckinghamshire in 1753. In 1764 Browne took on the post of Chaplain at Morden College in Blackheath London. He remained vicar of Olney at the same time as vicar of Sutton Lincolnshire until his death in 1787. Nel 1980 usciva presso la Nuova Italia Scientifica Il nuovo servizio psichiatrico, un’opera collettiva di Bruno Benigni, Fabrizio Ciappi, Giuseppe Germano e Paolo Martini, un amministratore e tre tecnici che avevano fatto la storia della riforma psichiatrica in Italia. In particolare essi riflettevano su quello che si sarebbe, poi, definito il “modello aretino”, cioè assumevano l’eredità di una della cosiddette “esperienze avanzate”, che negli anni Settanta del secolo scorso aveva precorso la legge 180, e rilanciavano la “questione psichiatrica” nel più ampio contesto della riforma sanitaria. Nell'introduzione gli autori scrivevano: “Il problema che si pone oggi è quello di rafforzare e di estendere lo schieramento di forze che ha voluto il superamento del manicomio, producendo un movimento che investa capillarmente il modo di essere dell’organizzazione sociale, con la consapevolezza che la legge conquistata è un punto alto nello sviluppo civile del paese”. Era chiaro per loro che il movimento per il rinnovamento della psichiatria doveva continuare ad interpretare il divenire sociale, cioè essere parte di quel processo democratico che aveva investito le istituzioni dalla fine del conflitto mondiale. Il “modello aretino” continua a funzionare egregiamente a distanza di tre decenni (lo attesta il sistema di valutazione MeS della Scuola S. Anna di Pisa collocando i servizi di salute mentale della nostra Area Vasta Sud Est, che più lo hanno applicato, ai primi posti della “classifica” regionale toscana); per la verità ha avuto una qualche egemonia più su scala nazionale (nel determinare il modello organizzativo sancito dal Progetto Obbiettivo 1992-94 con i “famosi” quattro livelli di assistenza, CSM, CD, SPDC, SR) che regionale, dove è stato poco studiato e poco “sponsorizzato” in sede amministrativa. Si può dire che il percorso non è per niente compiuto non solo perché l’applicazione della legge di riforma è disomogenea sul territorio nazionale (e per alcuni versi anche su quello toscano) e neppure perché è periodicamente insidiato da chi guarda al recente passato (le cosiddette “controriforme”), ma soprattutto perché quella che chiamo “l’istituzione diffusa”, cioè la rete dei servizi di salute mentale presenti nella comunità, è ancora distante dal diventare un’”istituzione della modernità”, per dirla con la definizione di Giddens (1994). Non è diventata su scala planetaria una riorganizzazione spazio-temporale della diversità psichica, riconosciuta universalmente sotto il profilo scientifico, culturale e sociale. Nonostante il suo lento decadimento, l’istituzione ancora prevalente sul pianeta è il manicomio, che ha siglato col “grande internamento” (Foucault, 1976) l’inizio dell’evo moderno, fatti salvi un paio di stati del Canada e degli USA, che hanno seguito la nostra strada del superamento per legge, e poche altre situazioni dove il superamento è avvenuto de facto, ma non de iure. Di questo la psichiatria italiana ha qualche responsabilità: da un lato la “litigiosità” ideologica dei sostenitori della riforma (Asioli, 2009), che ci ha di fatto inibito la leadership su scala planetaria e dall'altra la miope insipienza della nostra accademia, che – salvo qualche lodevole eccezione - ha perso un’occasione storica.
