Also published 'On asphyxia and on the resuscitation of still-born children' London Medical Gazette vol. 29 5 November 1841: pp. 222-27. / Between 1839 and 1841 Snow experimented with a guinea pig suffocating the creature and then beginning a dissection. He found that an hour after death that he perceived a 'slight vermicular motion in the right auricle. He opened the trachea and began artificial respiration. The heart's ventricles began to move and through the coast of the left atrium the chamber that receives blood from the lungs he could see oxygen-rich bright red blood. The heart continued to contract weakly unable to expel blood from its chambers but it kept beating rhythmically for forty-five minutes . . . This particular experiment took place in the course of his investigations into respiration and asphyxia undertaken with the desire to establish the physiological basis for pulmonary resuscitation on infants.' Snow was witnessing one in twenty births being stillborn many of whom were asphyxiated. Many methods of resuscitating were tried including electrical shock mouth-to-mouth resuscitation etc. 'snow surmised that the line between life and death was not fixed and the heart retained its irritability its ability to be stimulated by oxygen beyond death.' With this study done Snow's recommendation was to use his 'artificial respirator on still-born infants.' p. 1-3. This whole effort was to reinforce Snow's experimental method to study a medical problem. Because of this experience he was encouraged to continue his research practices. The announcement created a varied debate wherein many opinions and experiences were expressed. Snow's first real paper on ether anesthesia containing the first illustration of his regulating ether inhaler the earliest such device to control the amount of ether vapor received by the patient. Snow's paper published on 19 March 1847 appeared prior to his separately published pamphlet On the Inhalation of the Vapour of Ether in Surgical Operations October 1847; it was preceded only by three small tables on ether saturation of air that Snow published in the Medical Times London Medical Gazette and The Pharmaceutical Journal in January and February 1847. </p> <p>When ether anesthesia was introduced to England in late 1846 Snow immediately began experimenting with the process; he eventually became the first physician to limit his practice to anesthesiology. As the earliest specialist in clinical anesthesiology Snow was also the first to perform experiments on the physiology of the anesthetized state the results of which laid the foundations for the development of anesthesiology as a science. John Snow 1813-1858 of London was the first qualified physician to devote his full time to anaesthesia. Through his medical training he was interested in gases and when news arrieved from America in late 1846 that surgery could be performed painlessly by inhailing the vapour of ether he began his practice in anesthesia and his investigations. Since the first attempts to produce anesthesia in England with ether were inept some distrust of the method developed. Snow believed it was the technic of administrations and procceeded to remedy the situtation by developing an improved inhalter. Thenceforth the administration of ether was successful. Snow was not only a praticing anesthetist but being endowed with intellectual curiosty he was also a teachter and an investigator. Snow reported his new inhaler mask to the Westminster Medical Society. In addition he appreciated the need for measuring the ammount of agent administered the significance of oxygen lack the problem of carbon dioxide; and for the guidance of clinicians he divided the progress of anesthesia into five stages. All this material and carious researches were gathered into a monograph in October 1847. It was entitled "On the Inhalation of the Vapour Ether". It was and is a rare classic. Today there are only six original copies extant." "The Case Books of Dr John Snow edited by Dr Richard Ellis himself an anaesthetist is a fascinating record based on John Snow's three manuscript Case Books in the Library of the Royal College of Physicians of London. The importance of this manuscript material is that in this country at least they comprise almost the whole of Snow's existing manuscript material. The Case Books run from 17 July 1848 to 5 June 1858 the year Snow died. Their relevance is largely in connection with his activities as an anaesthetist and to a lesser degree as a general practitioner. From the manuscript emerges much previously unused material about Snow's pioneer work as a practitioner of anaesthesia in the earliest years of the specialty. Dr Ellis who died in May 1995 produced an absorbing work prefaced by an introduction of some 35 pages by himself and by a note on the prescription records by M P Earles. The preliminary pages also contain a glossary of Latin abbreviations used in the prescriptions. A transcription of the original Case Books occupies the greater part of the volume-550 pages. The end material includes a fascinating appendix of entries in Snow's own hand containing passages in which certain words have not been possible to decipher despite the valuable help from Dr Anne Summers Curator in the Department of Manuscripts at the British Library. Then follow some useful indexes: Index of Dentists; Index of Surgeons and other Medical Men; Index of Patients and Others; Index of Medical Conditions; Index of Places. From the pages of this publication Snow's activities spring to life. For example his entry for Thursday 7 April 1853 records his first administration of chloroform to Queen Victoria at the birth'. "Wendy Savage JOURNAL OF THE ROYAL SOCIETY OF MEDICINE 89 May 1996 pp.273-274 John Snow 1813-1858 English physician and a leader in the development of anaesthesia and medical hygiene. He is considered one of the founders of modern epidemiology in part because of his work in tracing the source of a cholera outbreak in London's Soho which he identified as a particular public water pump.
