L'opera cui è legata la sua fama è La psicologia del sesso, in sette volumi pubblicati tra il 1897 e il 1928. E’ considerata la prima trattazione sistematica della sessualità poiché, rispetto alle pubblicazioni precedenti, non si limita a considerare il versante della psicopatologia ma abbraccia anche il comportamento normale affrontando argomenti di carattere storico, antropologico, medico, oltre a fornire una serie di dati ottenuti attraverso questionari e interviste. Ellis introdusse il termine autoerotismo e analizzò con rigore scientifico il tema dell’omosessualità.
