Mai 1909 in Köln; ? 3. Januar 1987) war ein deutscher Neurologe, Psychiater und Hochschullehrer. // ? Die ebenfalls zu den epileptischen Äquivalenten im weiteren Sinne gehörenden epileptischen Verstimmungen zeigen im Gegensatz zu den gleichen Bildern rein psychopathischer Genese stets auch eine (wenn auch meist nur leichte) Einschränkung des Wachbewußtseins und seiner differenzierenden Leistung. Der Nachweis einfühlbarer Motive spricht im übrigen nicht gegen ihre epileptische Genese: seltener sind sie beim symptomatischen, am wenigsten beim posttraumatischen Krampfsyndrom beobachtet, Anfallsleiden, die erfahrungsgemäß weit weniger rhythmischer Regelmäßigkeit unterliegen. Beim genuinen Epileptiker findet sich nun eine periodisch vorwiegend mißmutige, weniger echt depressive Stimmung. In der Abgeschlagenheit und müden Unlust kenn-zeichnet sich die vegetativ-affektive Grundhaltung reizbarer Schwäche und ängstlich egozentrischer Selbstbeobachtung. Auch hier findet sich in den subjektiven Angaben wieder eine unbegründbare, oft angsthafte motorische Unruhe: im Gefühl des "Geladenseins" und der inneren Spannung kommt es nicht nur zu (gelegentlich vorangemeldeten) Attacken auf die Umgebung, sondern unter tief ergreifendem Mißmut auch zum Suicid; Sinnestäuschungen als Einsprengsel aus der sensorischen Sphäre scheinen hier mitzuwirken. Die vegetative Unausgeglichenheit findet in Tachykardie, Ohrensausen, Kopfdruck, Augenflimmern, profusem Schwitzen, spastischem Bauchschmerz und im Gefühl des Berstenmüssens ihren Ausdruck. SIEMEELING (1895) beschrieb auch deutliche Pupillen-änderungen (Starre). Subjektive Erlebnisse von Umgekehrtsehen, Kleinerund Größerwerden der Gegenstände, ihres Wegrückens oder Näherkommens, lassen sich von gut beobachtenden Kranken nicht nur in der Aura, sondern auch in Phasen der Verstimmung erfragen. Gerade der differenziertere und intelligente Epileptiker kann übrigens mit diesen Zuständen leidlich fertig werden und seinen Mißmut vor der Umgebung verborgen halten (RAECKE 1903). Ebenso unmotiviert und plötzlich wie sie kam, pflegt sich eine solche Stimmungsschwankung wieder aufzulösen; auch hier manchmal über den Umweg eines großen Anfalls, dem sie aber auch an Stelle der postparoxysmalen Schlafphase folgen kann. Es ist beobachtet, daß solche Phasen an die Menses gekoppelt, dem später auftretenden Anfallsleiden im regelmäßigen Rhythmus vorangingen (FKISCH 1928). Gleiche prämenstruelle Stimmungsschwankungen beschreibt BOJDINI (1937) bei einem (wahrscheinlich eineiigen) Zwillingspaar, von dem die eine Schwester in wöchentlichen Abständen epileptische Anfälle, die andere (demente) Schwester lediglich depressive Phasen erlitt. Gleich den poriomanischen Attacken kann auch die Vehemenz der Verstimmung sich im Spontananfall (MOEHLL 1860) bzw. im rechtzeitig gesetzten Schock auflösen. Auch GETJHLE (1930) äußert die Ansicht, daß "mit dem körperlichen Abreagieren" der Zustand vorbei sei. SCHULTE (1951) schildert graphisch einen Krankheitsverlauf, bei dem über 5 Monate ein vikariierendes Auftreten von (nachmittäglichen) Verstimmungs-zuständen mit Gereizheit und (meist vormittäglichen) großen Anfällen deutlich wird: unter Hydantoinmedikation weichen die Anfälle, an deren Stelle sich quälende Verstimmungen setzen, die nach Verringerung der therapeutischen Dosis und nach Auftreten gelegentlicher Anfälle wieder weichen.
