[WORLD WAR II / Royal Canadian Army Medical Corps (RCAMC.) / SIR HAROLD GILLIES] R. C. A. M.
Unofficial Diary of Plastic Surgery Division: 18 August 1944 - 11 July 1945"
United Kingdom: Privately Printed 1944-1945. Hardcover. Unpaginated pp. 129. Octavo measuring 8”x5”. Stiff card covers with green utilitarian backstrip. Autograph manuscript ink on lined paper bound notebook.

A singular manuscript diary of the first order: the compelling unofficial and deeply personal wartime journal of a senior Canadian surgeon covering the period from August 18 1944 to July 11 1945 offering a remarkably detailed view into the operations of a prestigious Canadian plastic surgery unit based in England during the final decisive year of the Second World War. Written with unusual at times wry candour it chronicles the devastating human impact of the conflict while bearing witness to the birth of modern reconstructive surgery and its techniques while conveying the harrowing psychological pressures borne by the military medical personnel tasked with the responsibility of rendering care to a relentless stream of casualties bearing a litany of complex physical and neurological injuries.

The diary is unequivocally set at the elite No. 1 Canadian Neurological and Plastic Surgery Hospital Hackwood Park near Basingstoke England corroborated by the official R.C.A.M.C. war diaries held at LAC/BAC 2. A highly complex endeavour the hospital served as an elite multidisciplinary hub maintaining extensive liaison between the various medical disciplines: plastic surgeons dental specialists radiologists laboratory staff neurologists and psychiatrists all working in collaboration to treat the often severely injured patients placed under their care. 1 Central to this network was the adjacent clinic of Sir Harold Gillies often credited as the “father of modern plastic surgery” at Rooksdown House Park Prewett Hospital. A pioneering figure Gillies trained a remarkable cohort of Canadian plastic surgeons who upon their return home adapted and refined his techniques profoundly shaping the trajectory of Canadian plastic surgery many of whom are referenced throughout the diary. Indeed the diary records frequent references to Gillies and his clinic noting many patient transfers between the two and documenting a close partnership that witnessed the rapid development and application of myriad pioneering surgical techniques honed under wartime conditions.

The hospital’s remit was not merely restricted to the care of Canadian and Allied soldiers. Indeed entries record care administered to a wide range of patients including Free French and Czech troops German POW’s and civilians bearing diverse injuries ranging from burns and fractures to complex head and facial trauma. Among the procedures documented are complex pedicle and skin grafts Nov. 20 1944 bone grafts for shattered jaws Jan. 9 1945 a hitherto unheard of toe-to-hand transplant Oct. 11 1944 the construction of prosthetic cheekbones for severe facial injuries Sep. 19 1944 and early clinical adoption of innovative techniques such as shrapnel extraction using a large London-sourced magnet Nov. 18 1944. Each instance undoubtedly illustrating the extraordinary imagination technical know-how and logistical coordination required of the hospital’s surgeons who managed an overwhelming caseload: in excess of “500” admissions since D-Day and “1344” procedures completed by the end of 1944.

Beyond its clinical and administrative insights the diary captures the profound physical and emotional demands placed upon the caregivers who contended with an incessant stream of catastrophically injured patients. It documents medical crises including accidental overdoses infectious outbreaks a patient suicide and critical injuries exemplified by the breakdown of a beloved colleague “Scotty” Dec. 20 1944 and perhaps most poignantly the author’s own admission of suffering from an acute bout of depression which would compel them to go on a self-imposed “strike” by refusing further work. The entries also record frank observations of the interpersonal dynamics at play: conflicts among staff the ambitions of superiors and collaborative problem-solving under pressure thereby offering a uniquely-layered record of wartime medicine.

Adding yet further dimension to the historical significance of the diary and underscoring the unit’s strategic symbolic and national significance are remarkable entries recording visits from prominent wartime political dignitaries including the Duchess of Kent Mar. 21 1945; Lady Louis Mountbatten Nov. 27 1944; Canadian Cardinal Jean-Marie-Rodrigue Villeneuve Sep. 18 1944; Lady Astor mid-Nov. 1944; Lord Beaverbrook June 13 1945; Angus L. MacDonald Canadian Minister of Defence for Naval Services among others. Crucially the diary consistently references key figures in the development of post-war Canadian plastic surgery and the chief participants in the day-to-day activites of the unit namely: Dr. Stuart Gordon “Stu” Dr. Hoyle Campbell “Hoyle” Dr. Lyman Barclay “Lyman” Dr. Frederick M. Woolhouse “the film star” "O.C" undoubtedly a reference to Col. Robert Morley Harvie 1893- who was the Commanding Officer among others. 3

Although unattributed the manuscript is undoubtedly written by a senior Canadian medical officer serving within the Royal Canadian Army Medical Corps at Hackwood Park. The diarist’s unique vantage point reflects a position at the very heart of both medical and administrative operations - almost certainly a surgeon or senior officer within the Plastic Surgery Division. Entries demonstrate direct involvement in daily operative work patient management interdepartmental coordination and a demonstrable ease with senior medical staff and command indicating a high degree of professional responsibility and access.

The manuscript’s Canadian provenance and context is further bolstered by a wealth of military political and personal references. Operationally the diarist reports to Canadian Military Headquarters in London noting frequent liaison with other Canadian General Hospitals Nos. 4 8 9 11 12 18 and 22 C.G.H. and repeatedly addresses the treatment transfer and repatriation of Canadian personnel particularly R.C.A.F. airmen. The Canadian orientation is also reinforced by mentions of Hackwood Park Christie Street Hospital in Toronto and the London Sanitorium in Ontario. Moreover visits from prominent figures including Cardinal Villeneuve and Minister Angus L. MacDonald - highlight the unit’s symbolic and operational importance within Canada’s wartime medical efforts.

The diary is a continuous holograph manuscript written in a single consistent and highly legible cursive hand with the ink varying from black to blue. Subtle variations in the pressure and speed undoubtedly correspond to the emotional tenor of the entries. A remarkably well-preserved and detailed near-daily record of surgical operations hospital administration and wartime life until the dissolution of the unit in July 1945.

¶ An invaluable and foundational testimony combining technical detail with human narrative. Remarkable in its scope it documents the sheer scale ingenuity and tenacity of Canadian and Allied medical efforts during the Second World War providing an unvarnished and direct account of the early development of modern reconstructive surgery the grim operational realities of wartime medicine and the professional and personal networks of surgeons who would profoundly shape postwar medicine in Canada and abroad. Rare indeed. Privately Printed hardcover
