Gorsuny in Wien dass man durch subkutane resp. submuköse Einspritzungen von Vaselin Unguentum paraffini vielen schwierigen chirurgischen Indikationen auf sehr einfache Art genügen kann. So erzeugte er eine Hodenprothese bei einem wegen Tuberkulose doppelseitig kastrirten jungen Manne heilte eine seit langer Zeit bestehende allen chirurgischen Maassnahmen trotzende Incontinentia urinae bei einer Frau bei welcher der Sphincter vesicae und die ganze Urethra zerstört waren. Die günstigen Erfolge dieser Methode wurden bald von Halban v. Frisch und Kapsammer bestätigt. Aber schon kurze Zeit darauf berichtete Pfannenstiel der nach Gersuny's Vorgang versuchte eine aus ähnlichen Ursachen entstandene Incontinentia urinae zu beseitigen dass er nicht nur keinen Erfolg gesehen habe sondern dass die Vaselineinspritzungen eine gefahrdrohende Lungenembolie zur Folge gehabt hätten. Auch hat es sich möglicherweise in einem der von Kapsammer berichteten Fälle um eine Lungenembolie gehandelt. Ausser Pfannenstiel der auf Grund seiner ungünstigen Beobachtung vor den Vaselineinspritzungen warnte wurde der Methode ein anderer Vorwurf durch H. Meyer gemacht der auf Grund früherer Thierversuche eine schädliche Wirkung nach der über kurz oder lang eintretenden Resorption oder Verschleppung des Vaselins in andere Körpergegenden für möglich hielt. Beide Einwände sucht der Assistent Gersuny's Moszkowicz zu entkräften. Er weist mit Recht darauf hin dass die Versuchsanordnung Meyer's nicht einwandsfrei und mit der beim Menschen vorgenommenen Injektion weder der Methode noch dem verwandten Materiale nach ohne weiteres zu vergleichen sei. Wenn er dagegen meint dass die Lungenembolie Pfannenstiels dadurch zu erklären sei dass dieser statt Vaselin Paraffin vom Schmelzpunkte 40° ein solches vom Schmelzpunkte 45° verwandt habe so ist dies meiner Ueberzeugung nach nicht zutreffend. Im Gegentheil glaube ich dass das Gersuny'sche Vaselin noch leichter Lungenembolieen erzeugen kann als das Pfannenstiels da es im Körper seinem niedrigeren Schmelzpunkte entsprechend weicher sein muss. ." Die geschilderten Nachtheile lassen sich nun sämmtlich mit ziemlich grosser Sicherheit vermeiden wenn man nicht Vaselin resp. ein ähnlich hochschmelzendes anderes Paraffin verwendet sondern einen Stoff dessen Schmelzpunkt hoch über der Körpertemperatur liegt. Ein solcher Stoff ist das dem Vaselin chemisch nahestehende aus der histologischen Technik bekannte Hartparaffin. Die von uns in der J. Wolff'sehen Klinik schliesslich verwandte Sorte schmilzt bei 57-60° ist leicht sterilisirbar und kann mittels der nachher zu schildernden Technik unschwer eingespritz werden." H.
