Il paziente deve "imparare" che ciò di cui ha paura non è più una minaccia e che ciò che vuole non è più desiderato. Deve scegliere di lasciar perdere i vecchi desideri e sfidare le paure primitive, le paure di tradire e perdere l'altro interiorizzato. Se il disturbo è un adattamento allora bisogna affrontare i desideri e le paure ad esso associati per introdurre cambiamenti. L'obiettivo è venire a patti con le Persone Importanti e le Rappresentazioni Interiorizzate collegate al modo di fare problematico attraverso i processi di copia: solo lasciando perdere questi antichi desideri si può aprire una breccia verso il cambiamento. Lavorare con la famiglia che il paziente si porta in testa, guardando il mondo dalla prospettiva del paziente, è alla base dell'approccio ricostruttivo e interpersonale nella psicoterapia individuale sistemica e relazionale.
